        <p>Лапароскопическая холецистэктомия может сопровождаться синдромом послеоперационной тошноты и рвоты, риск которого зависит от особенностей пациента, вида вмешательства и применяемого анестезиологического пособия. Использование опиоидов является одним из факторов, способных повышать частоту этого синдрома, поэтому изучение безопиоидной анестезии и построение прогностических моделей для ранней стратификации риска сохраняют клиническую актуальность. Цель исследования – выделить наиболее значимые предикторы развития синдрома послеоперационной тошноты и рвоты у пациентов после лапароскопической холецистэктомии в зависимости от метода анестезиологического пособия и разработать математическую модель прогнозирования риска синдрома послеоперационной тошноты и рвоты. Выполнен вторичный анализ базы данных проспективного рандомизированного контролируемого исследования с параллельными группами, включившего 120 пациентов American Society of Anesthesiologists II–III после плановой лапароскопической холецистэктомии на базе КГБУЗ «КМКБСМП им. Н. С. Карповича» (г. Красноярск). Пациенты были рандомизированы методом непрозрачных запечатанных конвертов в соотношении 1 : 1 в группу безопиоидной анестезии (n = 60) и группу анестезии с применением фентанила (n = 60). Исследование одобрено локальным этическим комитетом ФГБОУ ВО КрасГМУ им. проф. В. Ф. Войно-Ясенецкого (протокол № 109/2021 от 16.11.2021); все участники подписали информированное добровольное согласие. Синдром послеоперационной тошноты и рвоты регистрировали в первые 24 ч после операции при наличии хотя бы одного эпизода тошноты, рвоты или позывов на рвоту. Основным методом статистического анализа для выявления независимых предикторов являлась многофакторная логистическая регрессия с пошаговым включением переменных. Дискриминантный анализ использовался как дополнительный метод для классификации наблюдений и оценки качества модели. Для оценки прогностической точности применяли ROC-анализ. Частота синдрома послеоперационной тошноты и рвоты в группе безопиоидной анестезии составила 6,7 % (4 из 60 пациентов), в группе с применением фентанила – 53,3 % (32 из 60 пациентов) (р &lt; 0,001). В итоговую модель вошли метод анестезии, возраст, индекс массы тела, курение, наличие сердечно-сосудистых заболеваний и дооперационная боль по ВАШ. Наибольшую предиктивную роль для развития синдрома послеоперационной тошноты и рвоты имели анестезия с применением фентанила и наличие сердечно-сосудистых заболеваний. Прогностическая модель показала высокую внутреннюю точность (AUC Area Under the Curve (площадь под ROC-кривой) = 0,965; чувствительность 91,4 %; специфичность 95,2 %). Соотношение числа событий на один предиктор в финальной модели составило 36 : 6  =  6 : 1, что находится в пределах приемлемых значений для логистической регрессии, однако указывает на необходимость осторожной интерпретации и обязательной внешней валидации. В данной выборке безопиоидная анестезия ассоциировалась со снижением риска синдрома послеоперационной тошноты и рвоты.  Построенная модель продемонстрировала высокую дискриминационную способность при внутренней оценке, однако требует внешней валидации в независимых когортах. Необходимы проспективные рандомизированные исследования с внешней валидацией модели.</p>

        <p>Laparoscopic cholecystectomy may be accompanied by postoperative nausea and vomiting syndrome, the risk of which depends on the patient's characteristics, the type of intervention and the anesthetic aid used. Opioid use is one of the factors that can increase the incidence of this syndrome, therefore, the study of opioid-free anesthesia and the construction of prognostic models for early risk stratification remain clinically relevant. To identify the most significant predictors of the development of postoperative nausea and vomiting syndrome in patients after laparoscopic cholecystectomy, depending on the method of anesthetic aid, and to develop a mathematical model for predicting the risk of postoperative nausea and vomiting syndrome. A secondary analysis of the database of a prospective randomized controlled trial with parallel groups was performed, which included 120 patients of the American Society of Anesthesiologists II-III after elective laparoscopic cholecystectomy at the N. S. Karpovich KGBU (Krasnoyarsk). Patients were randomly assigned using opaque sealed envelopes in a 1:1 ratio to the non-opioid anesthesia group (n = 60) and the fentanyl anesthesia group (n = 60). The study was approved by the local ethics committee of the V. F. KrasSMU. Voino-Yasenetsky (Protocol No. 109/2021 dated 11/16/2021); all participants signed an informed voluntary consent. Postoperative nausea and vomiting syndrome was recorded in the first 24 hours after surgery in the presence of at least one episode of nausea, vomiting or urge to vomit. The main method of statistical analysis to identify independent predictors was multifactorial logistic regression with step-by-step inclusion of variables. Discriminant analysis was used as an additional method for classifying observations and evaluating the quality of the model. ROC analysis was used to assess the predictive accuracy. The incidence of postoperative nausea and vomiting syndrome in the non–opioid anesthesia group was 6.7 % (4 out of 60 patients), in the fentanyl group - 53.3 % (32 out of 60 patients) (p &lt; 0.001). The final model included the anesthesia method, age, body mass index, smoking, presence of cardiovascular diseases, and preoperative pain according to VAS. Anesthesia with fentanyl and the presence of cardiovascular diseases played the greatest predictive role in the development of postoperative nausea and vomiting syndrome. The predictive model showed high internal accuracy (AUC Area Under the Curve) = 0.965; sensitivity 91.4 %; specificity 95.2 %). The ratio of events per predictor in the final model was 36: 6  =  6 : 1, which is within acceptable values for logistic regression, but indicates the need for careful interpretation and mandatory external validation. In this sample, opioid-free anesthesia was associated with a reduced risk of postoperative nausea and vomiting syndrome.  The constructed model has demonstrated a high discriminatory ability in internal evaluation, but requires external validation in independent cohorts. Prospective randomized trials with external validation of the model are needed.</p>
