      <abstract xml:lang="ru" lang-variant="original" lang-source="author">
        <p>Агонисты рецепторов глюкагоноподобного пептида‑1 – семаглутид и тирзепатид – известны как сахароснижающие средства. Однако убедительно показано их благоприятное влияние на сердце, сосуды, почки, печень и нервную систему. Цель – систематизировать данные о плейотропных эффектах этих препаратов. Поиск проводили в базах PubMed и Scopus за 2016–2025 гг. Проанализировано 87 публикаций, из которых в список включены 38 наиболее релевантных исследований. Оба препарата снижают риск основных неблагоприятных сердечно-сосудистых событий на 14–26 % у пациентов с диабетом и на 20 % – у лиц с ожирением без диабета. Инсульт предотвращается эффективнее инфаркта миокарда (снижение на 16 % против 10 %). При сердечной недостаточности с сохраненной фракцией выброса семаглутид улучшает клинический статус, а тирзепатид уменьшает твердые исходы. Почечная защита подтверждена снижением альбуминурии на 26–46 % и замедлением падения скорости клубочковой фильтрации; в исследовании FLOW семаглутид снизил риск терминальной почечной недостаточности на 24 %. При неалкогольном стеатогепатите семаглутид обеспечивает разрешение воспаления у 59 % больных. Противовоспалительное действие (снижение С‑реактивного белка на 25–43 %) частично независимо от потери веса. Предварительные пилотные и поисковые данные указывают на возможное влияние на нейропротективные и аддиктивные исходы, однако эти эффекты требуют подтверждения. Плейотропные эффекты опосредованы активацией рецепторов глюкагоноподобного пептида‑1 в эндотелии, кардиомиоцитах, клетках почечных канальцев, гепатоцитах и нейронах. Тирзепатид благодаря дополнительной стимуляции рецепторов глюкозозависимого инсулинотропного полипептида дает большую редукцию массы тела (до 20,9 % против 14,9 %). Семаглутид и тирзепатид могут рассматриваться как мультиорганные кардио-рено-метаболические препараты.</p>

      <abstract xml:lang="en" lang-variant="translation" lang-source="translator">
        <p>Glucagon‑like peptide‑1 receptor agonists – semaglutide and tirzepatide – are glucose‑lowering agents. Recent evidence demonstrates their beneficial effects on the heart, vessels, kidneys, liver and nervous system. Aim – to summarise pleiotropic effects of these drugs. A search was performed in PubMed and Scopus (2016–2025). Of 87 analysed publications, 38 most relevant trials were included. Both drugs reduce major adverse cardiovascular events by 14–26 % in diabetic patients and by 20 % in non‑diabetic individuals with obesity. Stroke prevention is more pronounced than myocardial infarction (16 % vs 10 % reduction). In heart failure with preserved ejection fraction, semaglutide improves clinical status, while tirzepatide reduces hard outcomes. Renal protection is confirmed by 26–46 % albuminuria reduction and slowing of estimated glomerular filtration rate decline; the FLOW trial showed a 24 % reduction in end‑stage renal disease with semaglutide. In nonalcoholic steatohepatitis, semaglutide resolves inflammation in 59 % of patients. The anti‑inflammatory effect (C‑reactive protein reduction by 25–43 %) is partially weight loss‑independent. Preliminary pilot and exploratory data suggest a possible effect on neuroprotective and addictive outcomes; however, these effects require confirmation. Discussion. Pleiotropic effects are mediated by activation of glucagon‑like peptide‑1 receptors in endothelium, cardiomyocytes, renal tubular cells, hepatocytes and neurons. Due to additional stimulation of glucose‑dependent insulinotropic polypeptide receptors, tirzepatide provides greater weight loss (up to 20.9 % vs 14.9 %). Semaglutide and tirzepatide may be regarded as multi‑organ cardio‑renal‑metabolic drugs.</p>
