В проспективном сплошном сравнительном исследовании проведена оценка соотношения иммунного статуса с невротическими нарушениями у пациентов с хроническим вирусным гепатитом. Обследовано 110 пациентов: с активной HCV-инфекцией - 58 пациентов, с активной HBV-инфекцией – 27 пациентов и 25 пациентов с активной HCV-инфекцией в сочетании с серологическими маркерами HBV в виде «изолированных» Hbcor АТ. Пациентам антивирусное лечение не проводилось. Высоким показателям депрессии соответствовали увеличенная базальная и стимулированная продукция мононуклеарами цитокинов – ИФН-γ, Ил-6, большее количество в крови лимфоцитов CD56+. Уровень депрессивных признаков положительно коррелировал с базальными значениями ИФН-γ, Ил-6, количеством пула лимфоцитов CD56+ и отрицательно коррелировал с ФНО-α. (базальной и стимулированной продукцией). Большие значения шкалы ипохондрии ассоциировались с увеличенной концентрацией в супернатанте мононуклеаров ФНО-α, с высоким индексом стимуляции Ил-10. Установлены средние и сильные положительные корреляции шкалы ипохондрии с ФНО-α, Ил-10. Выявлено увеличение базального уровня Ил-4 и индекса стимуляции Ил-4, ИФН-γ у больных с проявлением черт шизоидности и гипомании. Обнаружено повышение синтеза ИФН-γ как базального, так и стимулированного уровня у больных ХВГ с высокой реактивной тревожностью. Длительная персистенция вируса в организме активирует иммунную систему, способствует развитию патологических нарушений в головном мозге. На характер иммунного ответа, клиническое течение болезни, несомненно, кроме других обстоятельств, влияет генетическая детерминированность. Этот фактор, вероятно, определяет избирательную подверженность индивида к нарушению психической сферы при вирусных гепатитах.

Prolonged persistence of the virus in the organism activates the immune system, promotes the development of pathological alterations in the brain. The nature of the immune response, the clinical course of the disease, of course, in addition to other factors, affect genetic determinism.
