В статье представлен сравнительный анализ интраоперационных критериев и показателей, характеризующих течение раннего послеоперационного периода, безгазовой лапароскопической радикальной простатэктомии (БЛРПЭ) и лапароскопической радикальной простатэктомии (ЛРПЭ). Всего было прооперировано 67 пациентов с раком предстательной железы, из них 35 (58 %) пациентов прооперированы ЛРПЭ, а 32 (48 %) пациента с использованием методики БЛРПЭ. Выявлено, что при использовании методики БЛРПЭ по сравнению с ЛРПЭ достоверно снижается объём кровопотери, длительность самого оперативного пособия, объём израсходованного СО2 и минутный объем вентиляции легких, а также у пациентов после БЛРПЭ по сравнению с ЛРПЭ уменьшается длительность госпитализации и частота встречаемости послеоперационных осложнений. Также применение БЛРПЭ позволяет полностью избежать развития гиперкапнии и респираторного ацидоза.
The article presents a comparative analysis of the intraoperative criteria and indicators for early postoperative period of gas-free laparoscopic radical prostatectomy and laparoscopic radical prostatectomy. A total of 67 operated patients with prostate cancer, from them 35 (58 %) patients operated laparoscopic radical prostatectomy and 32 (48 %) patient using the technique gas-free laparoscopic radical prostatectomy. It is revealed, that when using the technique of gas-free laparoscopic radical prostatectomy compared to laparoscopic radical prostatectomy reliably reduces the volume of blood loss, duration of the operation, the amount of CО2 consumed and minute volume ventilation, as well as in patients after decreases the duration of hospitalization and the incidence of postoperative complications. Also, the use of gas-free laparoscopic radical prostatectomy allows to completely avoid hypercarbia and respiratory acidosis.
