Статья посвящена актуальному вопросу современной ортопедии – корригирующим остеотомиям в области коленного сустава. Целью работы является обзор современной литературы для обобщения данных отечественных и зарубежных исследователей, определения показаний для выполнения корригирующих остеотомий, уровня и степени коррекции оси нижней конечности. Показаны результаты исследований влияния позиции механической оси нижней конечности на распределение контактного давления в суставе. Рассмотрены методики определения необходимой степени коррекции для достижения нормальной механической оси нижней конечности. Проанализированы литературные данные о результатах нормализации осевых деформаций в лечении деформирующего артроза коленного сустава. Применение данной методики целесообразно у молодых пациентов с гонартрозом второй или третьей стадии. Для получения хороших отдаленных результатов необходимо правильное и точное соблюдение показаний к выполнению корригирующей остеотомии, выполнение максимально точного расчета коррекции.

This article deals with an important issue of modern orthopedics – correcting osteotomies in the knee joint area. The aim of this research is to do an overview of contemporary literature in order to summarize the data from national and foreign researchers, to determine indications for correcting osteotomies, as well as the level and extent of lower extremity’s axis correction. The research shows investigation results regarding influence of the position of the lower extremity’s mechanical axis onto distribution of contact pressure in the joint. The authors consider techniques of determination of the correction extent needed to reach a normal mechanical axis of a lower extremity. Literary data on the results of axial deformation normalization in the treatment of knee joint arthrosisdeformans is analyzed. Application of the given technique is expedient in young patients with gonarthrosis of the II and III degree. In order to get good remote results it is essential to strictly adhere to the indications for carrying out a correcting osteotomy and to do a precise correction computation.
